Transport software testing
ends $1M defect penalties.

A state transport department was paying around $1 million a year in penalties for production defects. Within two monthly releases, PinnacleQM's transport software testing had every defect tested, validated, fixed and retested. The department no longer incurs penalty charges.

The challenge

Penalties paid for
every production defect.

Production defects in the department's transport systems were costing around $1 million a year in penalties, on top of the impact on passengers and services. The department needed to stop defects reaching production and clear the backlog affecting live services.

The department needed disciplined defect management and regression that would validate every defect's impact, confirm each fix and prevent regressions, within the existing monthly release cycle. It could not pause change while the backlog was cleared, so assurance had to work alongside ongoing releases.

What made it hard

Penalties on live defects
Every production defect carried a financial penalty, around $1 million a year in total, so each release carried real cost.
Monthly release pressure
Fixes had to be tested and released within the existing monthly cycle, without slowing other planned change.
Our solution

Every defect tested,
fixed and retested.

PinnacleQM introduced structured defect management and release assurance. Every defect was tested, its impact on connected processes validated, and each fix retested before release, so production issues were resolved permanently rather than moved around the system.

Regression testing protected each monthly release, confirming that fixes worked and that connected transport processes still behaved correctly before changes reached production. Within two monthly releases, every defect had been tested and resolved, and the department no longer incurred penalty charges.

How it was delivered

Two monthly releases
to zero penalties.

The recovery was completed within two monthly release cycles.

  1. 01

    Design

    • Defects assessed

Every open production defect was reviewed and its impact on transport services and connected processes validated before fixing.

  2. 02

    Execution

    • Fixes tested

Each fix was tested against the original defect and every connected process it could affect before release.

    • Retesting

Fixes were retested until they passed, and only then included in the next monthly release to production.

  3. 03

    Verification

    • Regression

Regression testing protected each monthly release from new failures, so fixes did not create fresh penalty-bearing defects.

    • Penalties stopped

After two monthly releases, the department no longer incurred penalty charges for production defects, saving around $1 million a year.

The benefits

What the
department gained.

Stopping penalties also protected passengers and public confidence.

  • Around $1 million a year in defect penalties eliminated.
  • Production defects resolved within two monthly releases.
  • Every fix validated against its full impact.
  • Regression protection for every subsequent release.
  • Fewer disruptions for passengers and operations teams.
  • Stronger evidence for release decisions.
